This form acts as an authorization to provide repair services to your computer based on our standard rates, or warranty status.
Apple warrants its products against defects in materials and workmanship under normal use for a period of one (1) year from the date of purchase by the original end-user purchaser. Repairs are covered under warranty when product hardware fails due to issues with materials or workmanship within one year of the product purchase date. The AppleCare Protection Plan and other AppleCare extended service and support agreements extend repair coverage for entitled products. Regardless of Warranty or Extended Service Agreement status, Apple excludes from coverage all repairs pertaining to user abuse or accidental damage. Warranty coverage does not include software repairs, data backup or consumable parts.
Foundation is not responsible for loss of, damage to, or corruption of data. We take every effort to maintain the integrity of your data throughout the process, however any irreplaceable data should be backed up prior to the computer being dropped off for repair.
In the event of a declined repair, or situation where no trouble has been found, a minimum diagnostic fee may apply.
All repairs require payment prior to parts being ordered, or at the time of pickup for labor-only repairs.
Repairs must be picked up within 14-days of completion. Foundation is not responsible for hardware left after that 14-day period.
Repairs falling outside of Apple’s Warranty or Extended Service Agreement will be at our standard rates for repairs.
